02 / Before you start

What you will need on hand

Five things. Gather them now and the form takes about 15 minutes. Hunting for them mid-form is the main reason applications get abandoned.

  • 01 Your EIN, and which proof of status you hold. The nine-digit employer identification number, written like 12-3456789. Proof of status is one of three: an IRS determination letter, a 501(c) application you have filed and are waiting on, or your state business registration. The last step asks you to confirm you hold one of them. If you are fiscally sponsored, you will also need your sponsor's name.
  • 02 Your annual operating budget, as a range. You pick a band from a list. We never ask for an exact figure, and we do not ask for bank or payment details anywhere in this form.
  • 03 Your current website address, if you have one. No website is a normal answer. There is a checkbox for it and it does not count against you.
  • 04 Links to anything you want us to read. Social pages, your 990 on ProPublica, your IRS determination letter, a Google Drive or Dropbox folder. Paste the links. Set sharing so a stranger can open them.
  • 05 About 15 minutes. There is no timer and no session that expires. Your answers are saved in this browser as you move between steps, so you can close the tab and come back.

03 / The application

Seven steps. Nothing we do not need.

Every field marked required has to be answered. Everything else can be left blank. We check your answers when you press Continue, not while you are typing.

Step 1 of 6 Organization identity

Organization identity

Who you are on paper. If your legal name and the name people know you by are different, give us both.

The name on your sign, your shirts and your donation page, if it is not the one above.

Your nine-digit employer identification number, written like 12-3456789. It is on your determination letter and on your 990.

Pick the one that is true, not the one you think we want. The program covers 501(c)(3), (c)(4), (c)(5), (c)(6) and (c)(19) organizations, organizations operating under a fiscal sponsor, and organizations waiting on an IRS determination.

Read the eligibility criteria

Four digits. An approximate year is fine.

Contact

One person we can write to. We reply by email. Nobody calls you to close a deal.

Executive director, board chair, volunteer coordinator, whatever fits.

The decision goes here. Use an address you check.

Only used if email bounces or something is unclear.

Can you enter into agreements on the organization's behalf? Required
Mission and programs

You are the expert on this part. Write it the way you would say it to a neighbor, not the way you would write it for a grant panel.

Paste the official one. About 300 characters is the sweet spot for a homepage. Going over will not stop you submitting.

0 of 300 characters

Programs, services, a normal Tuesday. This is usually more useful to us than the mission statement.

Describe the group. Do not send us anything identifying about individual people you serve. We do not want it and we will not store it.

A county, a city, a region, a state, or nationwide.

An estimate is fine. A round number is fine. Leave it blank if you do not track it.

Headcount, full and part time. Zero is a common answer.

An estimate is fine.

Scale and capacity

This program is for organizations with a budget under $1,000,000. A small budget is the point, not a mark against you. We ask for a range, never an exact figure, and never for bank or payment details.

Your most recent full year. Pick the band it falls in.

"Nobody" is the most common answer we get. It tells us how the site has to be built, not whether you deserve one.

A name and role. It does not have to be you. If you do not know yet, write "not decided".

An honest answer here changes how we build it. "Nobody yet" is a real answer and we plan for it.

Current web presence

Whatever you have now, including nothing at all. Nothing in this step disqualifies anyone.

Include the https:// if you can. If you do not have a site, check the box below and this field switches off.

The software the current site runs on. "No idea" is on the list for a reason.

The domain is the address itself, like yourgroup.org. "Unknown" is common and it never blocks an application. We flag it and help you track it down if you are awarded.

Be blunt. "Nobody can find our hours", "the donate button has not worked since 2023", "we have no site at all" are all useful answers.

Do you have any of these to share? Optional
Fit

The last step. The first question is the one we read most carefully.

Recruit volunteers without a phone call. Take a donation at 11pm. Stop retyping the pantry schedule into Facebook every week. Be specific.

An event, a campaign, a grant deadline, a season. Builds run 4 to 6 weeks from award once your content is in hand, so tell us the date and we will be straight with you about whether it fits.

May we name your organization and show the finished site as a portfolio piece? Required

Answer either way. Saying no does not affect the decision on your application, and we do not read it as a mark against you. You can also change your mind later: tell us in writing and we take your name down.

Review and sign
Acknowledgments Required

All five, please. Plain language, no small print.

Typing your name here stands in for a signature on the acknowledgments above.

Filled in for you. It is the date you submit.

Nothing is sent until you press Submit. If the send fails, your answers stay on this page and in your saved draft, and you can email info@omen.services instead.

04 / Questions
